The 19-year-old female driver was taken by ambulance to Hereford's County Hospital and treated for head injuries. Both teenagers are from Herefordshire.
The A44 was closed between the Forest Inn junction and Llandegley until 11.41 on Wednesday night.
Anyone with information that may help the police investigation is asked to contact Dyfed Powys Police on 101 and leave a message for PS 259 Westlake of the Mid Wales Roads Policing Unit.
